Downloading Adobe Acrobat and PDF files

If you click on the above link, you'll download a file with the form you have selected. If Adobe Acrobat Reader is linked to your browser, the page will appear in the browser window. Otherwise you will be asked where to save the file that is being downloaded. Note the name of the directory on your computer to which you download the PDF. The PDF file will provide you with a fully formatted version of the form.

To read a PDF file, you need Adobe Acrobat Reader on your computer. If you do not have Adobe Acrobat Reader, just click on the yellow icon below and it will take you to Abobe's site where you can download it for free.
